                                  Hospice, at your service

                                  Hospice Care aims to neither prolong nor hastens death. Our goal is to improve the quality of the patient's life by offering comfort and dignity.

                                  Franschhoek Hospice has a dual service, providing Palliative Care as well as general Home Nursing. 
                                  Home-based Care (taking place in the home of the patient) is provided for all the needs of the dying as well as caring for those with acute / chronic conditions.
                                  As we do not have an inpatient unit, we usually refer patients to either Luthando, Paarl or have them hospitalised to give the primary caregivers a respite.
                                  When possible Stellenbosch Hospice and Helderberg Hospice in Somerset West accommodate our patients.

                                  Empowerment
                                  During the first meeting, the assessment/evaluation meeting a registered nurse meets the patient and/or significant others. They are then informed on what Hospice is, what we do and who the Interdisciplinary Team (IDT) is.  The latter consists of employed and volunteer members from the different professions that may be needed in a patient’s care,  These include a doctor, a minister of religion, a physiotherapist, a dietician, a clinical
                                  psychologist, a psychiatrist, a social worker and registered nurses.Most significantly, the IDT is always headed by the patient

                                  Admission criteria

                                  A new patient must be referred by their GP / Clinic / Hospital Doctor. 
                                  A consent form must be completed by the patient and their family before Hospice can become involved.

                                  Policy on access to care
                                  Franschhoek Hospice provides the referral sources and the community with information on the Palliative Care Service rendered by Hospice.

                                  Hospice Care is available to all persons in need of Palliative Care in line with the admission criteria in all the communities of Franschhoek.

                                  The criteria for patients to enter the service are based on the ability of 
                                  Hospice to provide the necessary services.

                                  The patients are admitted to the Care Programme by competent staff
                                  members who give the required information to the patient and family in a
                                  clear and understandable manner. Interpreters are used when necessary.

                                  The care is available 24hours a day, on a system that provides after hours care on-call.

                                  We have a network of partners who are used to assist and support us with  speciaflised services that sit outside of the standard services we provide. 

                                  The duration of care is for as long as the patient requires it.

                                  What is Palliative Care?

                                  It's defined as “The active care of patients with progressive, far-advanced,
                                  incurable disease”


                                  Palliative care principles are based on the total care of body, mind and
                                  spirit.  The goal of Palliative Care is the achievement of the best quality of life for patients and their families.

                                  Hospice is not a place; it is a special concept of care designed to provide
                                  comfort and support to patients and their families when a life threatening
                                  illness no longer responds to curative treatments.




                                  The World Health Organisation states:

                                  early identification and impeccable assessment and treatment of pain and

                                  We serve the whole of the Franschhoek Valley which includes Franschhoek Central, Groendal, Mooiwater RDP Homes, Langrug Informal Settlement, Bosbou, Maasdorp, Wemmershoke Saagmeul, Hollandche Moelen, Wemmershoek Dam and surrounding farms.
